If you’re wearing decent IEM’s, with good eartips, they cut background noise to the same extent as industrial earplugs, by approximately 26dB. The tips you need are triple-flanged, like earplugs, they’re what I have on all of my canalphones, and they block all significant background noise. If you get MeeAudio M7 ‘phones, they come with a full compliment of eartips, plus they use MMCX connectors with two cables, one with a remote, and you can add a Bluetooth unit at a later date.
£110 for the phones, with two cables and the full range of eartips:

the BT adaptor seems to be only available through MeeAudio at the moment, but they’re easy to deal with:

Theres a BT adaptor cable available for the cheaper M6 Pro ‘phones as well. I’ve got the Pinnacle P1 ‘phones, which are like the M7’s, except they have a diecast zinc shell, and are single driver, so I’d suggest the M7 Pro, or if those are a bit too pricey, the M6, the big advantage with the M7 is that you can use any MMCX cable, like the Lightning cable Shure make for their SE series ‘phones.
This is the replacement eartip set for the M6/7 and Pinnacle ‘phones, the triple-flange ones I use are the ones on the right-hand side, two sizes for different sized lug’oles.
