I know it's not specific but couldn't you get just a nice, bound note book to record the information in?

There are pre-printed ones for Munros and such too.
midlifecrashes pipped me to the post. Wainwright ones have illustrations and quotes etc so seems a good job.
That Wainwright one looks good. The National Trust also do one.
