Filling out section 4 about parents details, which order do you write the names?? Is it surname-first name-middle name? Or surname-middle name-first name?

Ha obviously I should be shipped abroad, make me someone else's problem. Not getting this form caught up in red tape again after having being declined after my counter signed photo didn't say 'Mr' on it. I thought that was somewhat ironic, never being mistaken for a woman before, well apart from that one time. 😯
