I know it's usually (well, used to be, the other way around, but is there an easy way to convert the brazed-on, fully enclosed, hose guides on my top tube to 'normal' cable end stops?

There is. I have one I needed for an old Inbred.

I drill out the threaded cable adjusters such as found on caliper brakes. Some don't need any drilling at all.
