[Closed] Using a CRM to capture information about teams

Has anyone setup one of the open source or paid for CRMs to collect information about teams?
I'm looking at a small project for the school and I've had the idea that I could use a crm with web forms for folk to register / add their details to.

Typically, there will be 1 person (captain) this will need to provide 1 set of information (name and address plus some information about qualifications etc) and then a series players that will need to provide a second set of info (skill level, expereince, etc).

Anyone done anything similar, or can point towards some software that could do this out of the box or with some low level dev work? TIA
