I had the same happen to a Superstar cog, only around 150 miles use - I sent it back, and they replaced with 2 of the stamped steel type - still using the first of the replacements and it has been brilliant in comparison

I'm seeing a pattern here...

Maybe try a better brand cog.

The teeth that are still there don't seem to be showing much wear.
Edit apart from the supershite one that needs stuffing up fruit's/ neil's/ octopron's ass.
