I have a DT Swiss 350 front hub i want to adapt from 15mm to 12mm.
It seems that a sleeve to adapt 15mm to 12mm is a lot cheaper than a set of endcaps.
Are there any disadvantages to this solution?
If your fork has a recess to locate the end cap a 15mm may be to big 21mm diameter (19mm for 12mm axle), so could interfere and not locate correctly. Depends on the design of the dropout.

I would only use original DT spare parts. DT Swiss only offers a sleeve to switch from 15 mm to QR.
I'd only use a sleeve if a proper end cap wasn't available. As there is for the 350 just get one of those.
The only way I could make my 240s MTB 15x100 hub (oversized large screw on caps) work with my 12x100 forks; 9mmx100 caps and machine the bore and od to suit (was around 20mm OD and didn't quite fit the dropout).
So you may be in a similar situation.
I fitted a 12mm sleeve to a 15mm DT front hub, had to dremel the original endcaps slightly to fit the dropouts in my road forks.
