I'm running a single speed setup with a roller style tensioner and to my eye the top of the chain is aligned yet the bottom of the chain seems to pull to the left of the roller and pop off the rear sprocket, I've tried every possible spacer position, could the chain be bent/twisted ? any help appreciated.

maybe the roller is misaligned, pushing the chain off the sprocket

i don't think it's the roller everything is brand new except the chain so i'm thinking that is the problem

there we go, cheers for that takisawa2 and the hanger and tensioner are straight as far as my untrained eye can see anyway

Chainline looks spot on, but I'm sure that mech hanger looks a bit out of line. Could be your camera though.
Is it a new frame ? Any possibility its had a knock ?

frame is brand new, no knocks that i am aware of it's probably just my camera work but would a bent hanger make my chain shift over to the left ?

mech hanger / roller looks not parallel to the sprocket to me as well

Try with the tensioner pushing up?

Useful tip- never assume anything is correct just because it's new, i've spent a good chunk of my working life straightening brand new mechs and hangers.(and even more bleeding brand new factory bled brakes.)
