Mainly as it's not yours, it's your employers bike until such time as they sell it to you. Could get a bit complex if you put all your own kit on it.

No, but i have heard tails were bike has been written on the forms instead of frame. But as above, be prepared to buy it at what ever cost if the job comes to an end before the scheme.

In the early days, I got a frame/fork/headset through work/Cyclescheme. I raised an eyebrow at the time, but employer/Cyclescheme/bikeshop didn't so I just got on with it ๐Ÿ˜‰ Don't think you'd get away with it now through one of the one-stop-shop schemes unless you had an internal scheme..... and a friendly one at that!

Technically you aren't supposed to, but I'd have thought lots of places would be ok with it if you asked nicely...

on the NHS scheme I reckon it's unlikely. As bristolbiker says - your own scheme works well...
