Lowepro used to be fine, and mine's still going strong, but I have no experience of recent models. Most recent case was from Crumpler and is excellent. Not sure whether they do something that fits you rrequirements but suspect that they might.

I have an Apex 20, it is clipped to the shoulder strap with a karabiner and then there is a double Velcro strap on the back that goes over the chest strap. I have had a few big off's and it has stayed on fine and the camera has been nicely protected. And it has a water proof cover.

If its a small camera the berghaus leatherman case is nice. It's slightly stretchy neoprene and fits my pentax nicely

they do some nice bigger cases too
