have seen a few that have jammed onto the steerer tube, went removing or adjusting need to be loosened and knock down to release the wedge from the steerer, Dmr conrod I had a while ago with horizontal wedges did something similar.

My mates got one on his dh bike or rather had as it was a pain to get tight enough not to twist on the steerer and had to be really really lent on. He's gone back to a normal stem and has no issues.
