When removing the rear brake, do I detach the hose from the caliper or the lever?

If you can, remove the whole thing as a unit - with most bikes you can simply unclip the hose then unbolt lever and caliper and just take it all off as one. Less messy and easier to re-use. HTH
Neither - don't remove the hose unless you want to rebleed the brakes. Or is the brake hose threaded thru the frame mounts so has to be disconected?
If so it doesn't really matter which end
The hose is threaded through frame mounts, so will probably do it from the lever.
