I’m three interviews in, including theoretical tests and practical processing demonstration. They have asked what my salary and benefits package needs to look like. I know I’ve asked for way above what they usually pay, but I’ve got skills way above their usual pay grade. Without being arrogant I’m worth what I’ve asked for.
How long to I wait before chasing them for an offer?
I've had job offers come 20 minutes after interview up to 11 days after interview. Depends how quick their HR/Recruitment teams are usually.
Did they say how long that they'd take to provide a response?
I'd say 2 weeks, or one week after they've said they'd let you know, is reasonable to ask for an update.
They may have hoops to jump through to get your extra salary, but they should be keeping you up to date with what's going on.
"I've had another job offer recently and would appreciate a time frame on when I should expect to hear from you, so I can make a suitable decision"
Obviously tidy it up and make it sound more endearing, but I've used this before and got the offer through in <3 days.
It can take a while - we usually try and give an indication of when to expect a reply, but sometimes stuff get's in the way. Recent appointment, we said it would be a good week before we could get back. We then struggled to get to see the other candidates, and in the end, couldn't, so have made an offer to the person we saw as they were 'appointable'.
Not ideal when only one turns up for an interview, but that's what's happening out there.

How long is a piece of string?
If it's a big company it might take them a while to get feedback from everyone and make a decision on the preferred candidate. It will probably also take them a while to get any offer agreed and signed off internally, especially if you've asked for above what they're expecting to pay.
If it's a small company I'd expect things to move faster, but they might be working to very tight financial constraints, meaning a larger than expected salary might take some juggling to put together. They might also be in the last throes of new fiscal year planning (even though it's almost February, it happens all the time) so may not yet understand where the budget/headcount comes from.
How long has it been, and what expectations did they set when you spoke to them?

Took me 2 months. But chasing can't hurt after a couple of weeks.

Tough love:
You might be worth what you want, but if there is no budget for it, there is no budget. Hiring manager might be thinking FFS and trying to find other people who are in budget and has forgotten that he owes you a reply. In which case who knows when you might hear.

Could it be that some sort of exceptional approval process is required to offer outwith the normal band/rate for the role?
I once got asked if I was still interested in a job that I'd interviewed for fully one year previously! Fortunately in the mean time I'd found something else.

We interviewed someone the other week on a Friday afternoon. She was offered the job that evening, contract signed Monday morning, flew with us to the USA Tuesday am to a project kick off meeting with a new customer. Took it all in her stride!

A civil service role I applied for called me up with an offer 15 months after my interview 😂
National Grid not a lot better, 4 months just to set a date for interview.
I had an awkward situation with my current job. I thought the interview had gone well but had a really long wait. I badly wanted the job to save 2.5 hours on my daily commute.
In the meantime I'd been asked to do a 1 day/week secondment to a client from the consulting firm i worked for. My boss had told me that I had to get this 100% right because she'd stuffed something up with the client in the past so the relationship needed repairing.
I remember walking to meet the client to sign a contract and calling the job I wanted to ask if there was any news. They wouldn't tell me, so I assumed they were waiting for someone else to accept an offer. So I had to go ahead and start the secondment. I did a day of induction, then the next week did the first day of the work, and only then did I find out that i'd got the job, over a month after the interview.
It turned out my new role needed a clearance check and at the time the department that does the checks was overloaded because a global event was coming up leading to a month backlog.
I did feel a little bit bad for stuffing things up for my ex-boss.
