At Palma (Mallorca) I use Centauro - off site and its great, no queues and fast frequent shuttle bus.
Any recommendations for Malpensa (Milan) or any specifically to avoid?

Have used Hertz and Avis through work, both on airport which is a bonus.

Hertz were very dear, and would not have paid their prices myself, but service was good.

Avis were slightly cheaper, but after a couple of good experiences they started to let themselves down: charging for prepaid fuel, adding on late fees for on time returns and generally being unhelpful. One time a friend booked through them on a cheap net deal and on returning the car to the airport, they 'found' some damage, which was textbook scamming. I have avoided them since then, and also at Linate where they tried similar scams on me.

Have used SicilyByCar a few times since, and they have been much cheaper, on airport (although slightly further away than Hertz and Avis) and provided very decent service. Their desk is a bit cheap and cheerful, and the cars slightly less fancy, but good enough.

They used to be massively cheaper if you booked through AutoEurope, but seemingly less so now.

One other point to watch, a lot of the filling stations near MXP close at lunch and in the evenings / night, leaving only the self service pumps open. However, many of these pumps refuse to accept non-Italian cards, so you can get stuck trying to refuel a car before returning it. Try and find a manned filling station or fill up on the auto strada as close as possible to the airport.

I've used the brokers Holiday Autos a few times and found the rates to be really low.I've always stayed away from the fuel pay up front and return empty rentals and had pick up full return full.Last time to Spain it was £23 for 23 days.

Literally just used budget (still at the airport waiting for return Flight). budget share the operation with avis. I booked via Opodo and got a reasonable deal. There was the usual why not upgrade for 20 Euro a day when I got here but same everywhere and no problems.

Well that was a faff and a half. Flight into T2, but my car hire collection was at T1 with a shuttle bus that takes you on a 20 minute mystery tour between them. It looks as though premium (gold card etc.) members of some of the big players (hertz, budget etc.) have pick up and drop off at T2 otherwise add an extra hour onto your journey to factor in car collection. I think I'll look at GVA next time.
