as above, just received an invoice addressed to "The Leaseholder" for the last 5 years ground rent on my property. I'm aware that I should pay ground rent, and the value is correct (£15 at £3 per year), however it's on a piece of plain a4 with no real details other than who the cheque should be made out and sent to. I've checked the title register and the person who is asking for the cheque is not mentioned on there. I know it's not a lot of money, but slightly concerned this could be some kind of scam. Would you pay it?

Definitely not.

Onus is on them to identify themselves properly.

You'll probably get a letter soon asking if you want to buy the freehold.
