Looking for a velcro / ratchet type shoe. Already tried and dismissed Shimano, Giro and Garneau as when they're long enough I can't get them tight.
Plan b is thicker / extra insoles to take up volume but a properly fitting shoe would be preferable.

Mavic shoes are pretty narrow if you go for the standard fit.
Or Shimano custom-fit?

I have exactly the same problem. Mavic are some of the best I've found for fit.

Sidi's, standard fit (not mega) are lovely and snug for narrow and shallow feet. Go for the one size up method, and they should be spot on
