Would I get away with a medium mech or have to put up with an ugly large thing dangling down behind the bike?

If running 1x10 then you'll be fine with a Medium cage. I'm running a medium cage Deore with no problems and the B screw most of the way out

I'm running that casette with a Deore mech with RAD cage, perfect shifting. Haven't tried it without the RAD cage though as I already had it.

Shimano GS medium 11 speed rear mechs are designed to work with that range cassette and by all accounts work flawlessly with 10 speed.

I've used medium and long cage SRAM x7 type 2 mechs, they're ok but the jockey drags on the 42t a bit. Is there something like the goatlink for SRAM to drop the mech a little?

Got a medium slx on mine with a Goatlink which helps it run with less b-tension.
