Looking at JRA they talk about 21mm and 25mm yellow tape. The stuff I've got, which was sold as Stans tape, is only 12mm wide.
What's going on? Has the tape Stans recommend changed over time?

Doesn't sound right at all.
21mm is for Olympic rims, 25mm for slightly burlier ones like Arch rims.
Perhaps 12mm is regular rim tabe, not stuff to seal your rims for tubeless purposes?

the 12 mm stuff covers the spoke holes under the rim strip. The 21 mm is for sealing narrow stans rims, the 25 mm for sealing Flow rims

Yep - as others have said you need the 25mm Stans yellow tape for Flow rims.
