My GF much prefers riding with flats, but currently makes do with trainer type approach shoes. We've been looking around for Vans or similar for her, but we can't find them smaller than size 5. She probably needs a size 4 / 4.5. Any ideas?

The FiveTen Karver womens shoes go down to size 3...
Pricey but definitely worth it IMO...

Alternatively, I remember reading an article somewhere (Dirt maybe?) a while back about a company that resoles any shoe you like with 5.10 sticky rubber.

Could buy some skate style shoes and convert them?

EDIT: and a link too... http://www.feetfirst.resoles.co.uk/cycling_mtb_shoe_repair.php

£35 isn't cheap, but guess if she already has some trainers that you could send off?

Vans go all the way down to childs size 3 or 4....

Try looking in Schuh (or however it's spelled) or TKmaxx. They usually have a selection, and are next door to each other in Sheffield.

Otherwise, feet first resoles in Woodseats are the company who will put 5.10 rubber on anything. Finally, Vans outlet stores are a good place, there's definately one at cheshire oaks, and I think there's one at Mansfield.
