How do you measure a modern MTB stem, is it the imaginary center-point of the bars to the center of the "star washer"?
Also what do I need to look out for in terms of handlebar fatness figures - are oversized bars any particular size?

yes. from mid point of bars to mid point of steerer.
As for bars - either old style 25.4mm (1") or newer 31.8mm (1.25")
