the bottom flat bit parralel to the ground and the brake levers vertical?

Mine are set up pretty much like that, but as you correctly say it's personal and dependent on bar profile. With the lower part horizontal the top bit behind the hoods slopes slightly downwards which feels good. See lots of bars/shifters pointing skywards but that just feels wrong to me.
bottom part parallel to top tube for a PRO AS look. Avoid STI poking your eyes out vertically like a sportive/novice/trianthelope.

Get the top part at an angle that's the most comfortable, its where you're going to spend most of your time.
Run the shifters higher than is deemed "cool", again its about comfort.
Avoid setting controls up on how it looks, arse in the air, chin on the tyre may impress some folk, but after 4-5 hours in the saddle, you'll be a mess.
Just look at the pro peleton to see how high up the bars some of those guys run their shifters...

Not sure you can climb out of the saddle on the hoods properly if too high on bars.
Check Belgian riders, long stems , flat backs......then again, what do belgians know (museeuw, boonen, gilbert), however, if everyone wants to look like emma pooley then fair play...it just looks shit thats all.
If you can ride 5 hours on a road bike then you probably have your bike set up properly anyway...unless your doing a sportive where the 5 hours probably covers 40 mile riding a dogma and looking like Wiggins in full kit.

That Trek is set up spot on because the frame looks like its too big for the rider, Scott looks OK though (apart from the Cervelo bidon's) ๐

set up with the bottom bit pretty horizontal, levers quite low down on the curve, as i was having reach issues if they were higher up (when on the drops)!
just been for a ski, and off for lunch, but 1st ride this afternoon to see how it all feels. was surprised at how painless the bar tape was - i'd imagined hours of rage!
