Can you do this without the BB special tool/spanner thingy?

Perhaps with one of those rubber strap wrench things (if it's external) but I seriously doubt it would have enough 'grab'

You can use a Mole Grip/pipe wrench but it will savage the shells.
You can pre-load with anything that will vaguely fit the nut...remember it is preload, not 20Lbs.Ft of torque.
At least the drive-side shell will be hidden by the crankarm from the tool police.
