For doodling with in a little book, probably a pocket size Moleskine or something. I like a Sharpie for drawing larger, but the line's a bit too chunky for a week book. Not too keen on Pilots and the like, too scratchy.

I find a half pencil fits nicely along the top or bottom of the little moleskin held by the elastic without protruding.
Cross. Very compact.
get thee to ikea, grab handfuls of free pencils, leave.
I buy boxes of 144 half pencils from ebay. They're littered all over the workshop/house/my ears. I love them 
I love Lamy Safaris. Either ballpoint, pencil and fountain work great, are ever so comfortable to use, look good and are sensibly priced.
The ballpoint refills last forever and don't blob. Which when I'm designing kitchens and bedrooms and do all my own drawings to show the customer is very important.
Those Fisher Space Pens are tiny, but i've got one and it's fairly unpleasant to write with.
Pilot do a half-length version of their gel thingy (g2?) that's rather good for that.
Ikea pencils, now there's a good idea!
Pen size isn't *that* important, it'll mostly get chucked in a bag, more interested in nib size. Though thinking about it, something that straps to the book would actually be pretty ace.
After discovering the hard way (middle of a 3 hour exam) that not not all rollerballs / gel pens are created equal, I now only buy uniball signo 0.7 jobbies - in a range of colours obviously.
uni-ball signo are quite good and do colours too. My personal favourite is a Lyreco Premium - a slightly fatter variation of the Pilot and writes well - but hard to find.
Have a look at space pens. I think that is what they are called, I had one a good few years back when they were all the rage at high school. They were pretty small and compact and may fit in a similar way to the half pencil. The lid was quite long which made the pen more comfortable when writing with it.

Ive got a space pen in the van. It's ugly to write with - really lumpy ink and nib. But the lid on the back does make it easier to handle
for me.
edit - and geoff, i too fell for these through exam usage!
