I'd suggest just behind and outside

Behind, and to her right – basically protecting her, and watching her and anyone else.

Not taken any children out on bikes, but I would have thought slightly behind and to her right.

    No personal experience though

Behind ,you protect her and can grab hold to steer her or if she forgets to brake,or even push her into the verge in an emergency

I always cycle behind my kids so I can see where they are going…or else you spend a lot of time weaving about as you're looking backwards. It also helps that you can keep talking to them, help them when pulling out for parked cars, positioning on the road, speeding up for upcoming hills etc and finally you can position yourself in the road so when cars pass you they leave a big enough gap for the kids.
But thats only my personal opinion
