    How much axle should be showing between external BB and drive side crank on a hollowtech II?

    as above. Your inner ring (on a triple) should actually be slightly around the BB cup.

    Do not be tempted to use the plastic pre-load cap on the non-drive side to pull it through. Push/tap it home from the drive side.
