I need to get a new rad with a btu rating of around 4500.
Looking at getting a column style rad but I've seen two which are very similar in size but have different btu ratings. 3400 compared with 4500. How is rad btu calculated?

Surface area and temperature of the water in it vs temperature of the room.
In your case comparing two radiators from the same range, the difference is surface area.
