I really like the Nirvana MTV Unplugged album and have just realised my collection is lacking much else that’s similar.
What should I listen to next? Not necessarily live, but a similar laid back pained vibe?

Few things out there in that form and that good. It’s a stone cold classic in every way.
Biffy Clyro Unplugged at the Camden Roundhouse. Effectively their tribute to the Nirvana Unplugged.

One of my favourite albums ever, such fond memories of my mum playing it in the house when I was young.
It's one of a kind though, so not really sure where to go from there.
If you like the laconic grungy sound, and don't mind lots of guitar noodling, try some Dinosaur Jr.

The Alice in Chains unplugged. Prefer the Nirvana one (just) because it had more nteresting covers but they are both exceptionally good.

Mark Lanegan. Friend and occasional collaborator with Kurt Cobain.
Great if you like the dark blues/folk thing.
Worth checking out the Meat Puppets and the other MTV unplugged sessions (Alice in Chains, Pearl Jam and Stone Temple Pilots) and early Mark Lanegan solo albums (whiskey for the Holy Ghost, The Winding Sheet and I’ll Take Care of You) as mentioned above. Chris Cornell’s Song Book album too. I actually prefer all of those to the Nirvana unplugged album.

If you’re enjoying him he has a huge back catalogue of solo work, band work and collaborations. I’m waiting for his biography to come out at the end of this month. Should be an interesting read to say the least!

The 10,000 Maniacs Unplugged equivalent is great too, much the same kinda (ish) laid back alt country vibe with female singer.

Try The White Buffalo-hogtied like rodeo, mellow but good. Also sublime did an acoustic album which although a fair portion sounds like it was recorded drunk at a party is good!
