so, painkillers kill pain, but using drugs has more side effects than not using drugs.

I guess that depends on whether or not you include being in excruciating flesh ripping eyeball popping pain as a side effect of not taking painkillers.
...then you have the 'midwife' factor. Not all are created equal.

Diamorphine (heroin) is available during delivery in the NHS. Hard to see a back massage competing with the big H for pain relief.

The only pain relief my wife used was a TENS machine, which she thought worked very well. We've no way of know whether it was the placebo effect, but that didn't seem to matter very much at the time.
Hard to see a back massage competing with the big H for pain relief.
