The ones on the road like the yellow lines one above can happen when they get driven over due to vibration and people turning their wheel while stopped on top of them.
When I worked for the council that was the [s]excuse[/s] er' reason.. we gave... 😳 some of em were really heavy to faff around with though, and sometimes it was just funny...

Jamie's light and dark paving lids...not surprised they are out of order!

The council replace our fairly light concrete lids with those heavy deep set things that are a pain (literally) to remove and stack, by rights we are not even meant to enter them as most are unapproved copies. They jam when removing them and underneath are absurdly heavy cross members nearly as large as the lids themselves 👿
