The bike wasn't in use but the reservoir cap has popped from the body of the brake and ripped the threads out of the lever body that hold the reservoir cap in place. The screw seems fine.

Has anybody every heard of anything like this happening?

I can't see it would happen with a bike unattended - unless you've boiled the brake fluid or forced the pistons back into the calipers very, very hard?

Well said John. youmaye buy second hand brakes, youmeya crash into a tree at speed on first outing, youmeya bend levers all the way round, youmeya mend them yourself, youmeya have big problem.
