Peli do a box that is iPhone size and has a headphone outlet. Seems to be waterproof
I've been through a few cases for my iPhone and like said by Sandwich the Pelican case is the most bulletproof but if you want to be able to use the phone without taking it out of the case I think the H2audio case is the best.. You can use it with an arm band or waist belt.. You can see both in this pic.

Had my iphone 4 permanently in a lifeproof for about a year, ace. Took it swimming once and was totally sealed, daren't do it again !
I've got one of those. I've no doubts that it's fully waterproof and you can use the touchscreen just fine, but it's sufficiently chunky that I don't really use it. Would be fine in a bag / big pocket / on a lanyard. Used once, yours for £6 posted if you want it.

Reasonably cheap, let's you work everything except the miute switch and has accessories that let you mount it on motorbike etc, you can also charge the phone while it's in the case. I've had one for nearly a year only negative is tending to lose the headphone socket plug if you unplug to put headphones in the phone it and leave it flapping.

Got one of those peli cases but has a 3.5mm port. Can't remember the number but used with waterproof remote headphones is perfect.
It's an i1015 case. Got a similar one for an iPod classic.
Also have a griffin Survivor case for iPhone; not fully waterproof but good for MTB.
Lifeproof case. really.
