They don't come with a head doctor or top cap if you get a pick n mix head set.

mine didnt have spacers, but did have a head doctor and top cap.
standard external 1 1/8.

As stated, if it's one of their packaged headsets you get the head doctor but no spacers. If it's a pic and mix then you don't get the head doctor either.

The head doctor is awesome, transferred mine between 2 sets of forks now and they just work.
