Recently I've started getting knee pain (left leg only) on the front inside of the patella after a few hours in the saddle. It presents itself as a fairly sharp pain after a few pedal strokes. When I'm not on the bike there is no pain.
Has anyone else experienced anything like this and if so what was the solution?

I think there's a relationship between whether the pain is at the front or the back of your knee and whether your saddle is too high or too low but I can't remember which is which...
dunno but what i find keeps my knees right is a pair of insoles with a bit more arch.if i get any pain i use the insoles untill it goes then swap back to my normal ones.
superfeet are the ones i have.
If the pain is in front of the knee, your saddle is too low, and if its behind, the saddle is too high.
Maybe you're pushing too big a gear?
Having said that though, its not usually a sharp pain, maybe have a look at your cleats too.
Also, make sure your feet are pointing the correct way! as in, toe in or toe out.
I've had mine worked up with MRI etc - Tried most things but now run on ibuprofen until I kneed knee replacements ๐ณ (see what I did there!)
I've got the same problem. Have had some relief from specialized insoles plus varus wedges but still have to concentrate on warming up gradually, pedaling in circles and spinning rather than mashing.

worth looking at bike setup - saddle height, fore and aft, cleat angle etc. I have had 3 knee arthroscopies over the yrs and keep things under control with careful setup, not pushing big gears (more likley to do this on a road bike), various glucocosamine and related supplements, spesh shoes and footbeds, time pedals and cleats, regular sports massage and monthly trip to osteopath. I also swim (front crawl) and probably have less problematic kness that I had a few yrs ago.
Are you clipped in? SPDs or other brands? Since I stopped clipping into SPDs on my commute my knee pain has gone.
Spin in lower gears and get your cleats setup correctly if you use them. The problem may actually be your knees and cycling aggravates this injury so if things persist I'd see the doc.
I've torn the cartilage in both my knees and get pain after several hours of riding. Ibuprofen keep me going for now until I get them fixed.
My knee pain disappeared overnight when I changed from SPD's to time pedals. I find they allow my feet much more lateral movement, rather than being totally static like with SPD's.
The problem seems to have come on quite rapidly. I've been mountain biking with SPDs for a few years fine.
Then about 6 weeks ago I borrowed a tourer on which I did 350 miles over four days again with no problems. Then last week I did a hilly 100K ride on the tourer and in the last hour I got knee problems. Initially I put it down to the tough ride.
However a few days later I did a 2 hour flat ride on the MTB and got the same problem after 90min.
I'm going to move the cleats back on the shoes to see if that reduces some of the pressure on the knee. If that doesn't work I guess it off to the physio, or are there any other health care professionals to consider? I'm sure the physio will diagnose the problem, but I doubt they'll be able to translate it into bike position.
