Bikepacking is not extreme, but sods law that you take a tumble on a gravel track and they fail to pay out.

Easyjet cover mountainbiking as standard last time I used them.
If you're not going gnarr (and staying within the EU), you might get away with just your E112 card. You do need insurance for places like the alps because some countries bill you for the helicopter flight to get you out of remote areas. Obviously doesn't cover things like repatriation costs either.
Snowcard should offer cover for what you want.
