when bivvying do you use your theramrest (or equivalent) inside the bivvy bag or outside?

outside, but thats largely cos I use a small tapered lightweight bag and It wouldnt go inside, Do sometimes roll off but im the same in a tent.

There is always a tendency to roll off it during the night if its not inside the bivvy bag.
