It’s the three prong UK adaptors for the transformers rather than the actual chargers. Six incidents of mains electric shocks is not to be sniffed at. iPad and presumably old WiFi adaptors too. Anything big that plugs into the UK mains. If you take off the adaptor and it’s white inside not grey, replace it.
I assume incident <> death, but not everyone has an rcd

Indeed it's for Mac and Mac books it's has 6 reported cases of them breaking, no idea where mine is as not used it for a couple of years.

It’s at least 7, then, because the same thing happened to mine. If you pull the body of the charger it stresses the plug along that seam you can see. Eventually mine came apart and yeah it reveals a couple of wires that would obviously give you a nasty shock. I just glued it back together and carried on with my life. ¯\_(ツ)_/¯

recalled what looks to me like same part down under (albeit with Aus/NZ prongy bits) well over a year ago - i was surprised by how many we had and was a 2mins in and out at local mac dealer no fuss
edit just looked it up Jan 2016 now go check travel box for UK ones

ipads used to come with a wall plug like that with a USB port with a higher amp output.
I've got one of those. Not sure if it's the recalled one... but if it broke and it was still plugged in, I reckon those "exposed metal parts" wouldn't be touched.
