Why do we need them?

What would happen if I didn't put them in with a: cartridge type eg hollowtech 1 or b: outboard bearing type? hollowtech 2. Are they needed for 68 and 73mm BBs?

Why can't they make the bb the right width so you don't need them?

Well, for a small width (68mm?) BB you need them to get the same chainline as a larger width BB. For a larger width BB you need the little bit of space in case you want to add a chain device or some such malarkey, then you remove the spacer. I think.
