I bought a 2nd hand set of spds and shoes a few months ago to give them a bit of a go on the road bike. The issue that I have with them is that the two tendons(?) behind each knee feel like they're on fire post riding with them, on the bike it's fine and with flats it doesn't happen. I'm guessing that this is probably an incorrect set up.
I've tried reading a few guides online but any changes that I've made haven't helped. Does anyone have a foolproof method for setting them up?
Also - the shoes are a bit too small, is there any way that this could be the issue? The shoes aren't small enough to hurt my feet but are small enough to know that they're a bit small.
Sit down on a chair with your feet off the floor, they will stick out at different angles from the stright ahead. So make sure your feet clip in and still have that amout of angle.
My left stick out a long way, my right much less - everyone will be different. Make sure you get this correct. If you cant get it right you will bugger up your knees.
You may have to use a different pedal if you cant get it to work, some allow more angle than others. Time and Crank bros allow more than SPD's I think.

Thanks for the tips Trimix, I had tried that and seems that both my feet point a bit outward, I did change my cleats to reflect that and it helped but didn't fix it. I am somewhat concerned about buggering up my knees which is why I currently have flats on the roadie but they just aren't as good!
