I recently picked up a Trek Stache (I have been banging on about getting one for ages) and it has the DT 350 rears hubs, an excellent hub but the 18t ratchet is a bit lame especially compared to my my 36t on the 350s fat hubs on my other bike. Before I go down the route of buying some 36 or even 54t ratchets I have questions.

Has anyone used the 54 and had any issues with reliability on them?

Does anyone have any spare before I look at buying some?

I think they can be a bit more inclined to slip if you're a big lump. I have the 36t and have no issues, not sure I'd go 54.

I've got 54T in my Stache and have not had any problems. I believe that 54T wear quicker though because there is less contact patch. Saying that, I've had a lot of DT hubs on road and MTB and I've never had an issue.
