A sh frame I'm building up has this...there appears to be no way of tightening if once it's in the steerer, do you just get it as snug as possible, insert and snug-up again by hand?

That's how they usually work. It has to be a right fit in the steerer to stop it spinning whilst pre-loading the headset.
PS - if you are struggling, take it to a LBS where a qualified person will fit it for a couple of jaffa cakes
It takes a quialified person to work that out?
