Do I have to ask the car hire company if they agree to it, or do I just say I'll pay the excess myself if a problem and not buy their insurance?

Just sort it yourself. Iirc, in the event of a claim you pay the excess and then claim it back from the excess insurance company, so the hire company don't even know that you've got the policy.
