If you have a 150mm hole already drilled and now want to run a 127mm pipe through it, I am sure I have seen a foam adaptor that does this nicely. I mean I could just space it with window spacers and expaning foam around, it but the one I saw looked neat.
My google-fu is waning, I used to be able to find anything.

Yeha this was like a 1m length of insulation ID127mm OD150mm that you cut to lenght and put in the hole. But both those links would do it easily too.
