Hi I have a medical condition that means it is difficult for me to press a normal gear lever, could anyone that has tried the new xtr di2 system tell me how hard you have to press the buttons to change gear?

Not hard at all.

Grip Shift, as a cheaper alternative?

about the same as an XTR rapid fire imho (I've got it) I'm not sure why they mimicked it so much

I've no idea if it's possible so this might not be helpful but what about the Di2 triathlon shifters as I think they are literally a push button.

my suggestion would also be to see if the tri or the sprint buttons work. as said they are literally push buttons so should be real easy to use.

sprint would need splicing as the cable end is designed to go into a port on the lever and doesn't fit into the junction box

climbing shifter would be easiest, but you cannot (or couldn't) use it on its own, as you need a shifter with a "brain"

Does anyone know if you can use the tri buttons with xtr and without an extra shifter?

"probably" if you only use the rear mech if you want 2x then you need the XTR shifter as the XTR front mech doesn't play nice with anything else (different firmware)
