I've just been told that someone has taken my KOM title. However, I wasn't aware that I even had this title. Is this down to someone newly creating a segment?
Probably. If you look at 'my segments' it shows all your KOMs.
Yes, that is my point. This particular KOM never appeared there.
I got two new KOMs last week despite not creating those segments. I expect it is for the reason you mention.
They should appear in your segments though. Most likely someone created it, it made you KOM, then as it uploaded all the history he took it.

I've reported myself to Strava.
I lost a few at the weekend because a team car at the Fleche du Sud had someones Garmin still on when they switched bikes. The big clue was the 110kmph speed they did uphill.
The KOMs achievements should show up in your profile unless someone else has a matching time and it's their name at the top of the list.
