How much extra is the minimum I need over headtube length for a fork steerer tube? (normal headset, couple of spacers)

3 inches at least i'd say (or 75mm)

Shortest stem 35mm, 40mm more common.
Think shortest headset 30mm IIRC

crank bros iodine headsets are 21mm stack height which is about as low as you'll get without having internal/integrated headsets.
don't forget you need about 5mm less than the combined height of headset,stem, headtube and any spacers for the steerer tube.
