I've had this problem also with an old Force CX1 level where if you dump a few gears and the shifter barrel rotates but the mech / cable don't, because the gear cable isn't captive it comes out of the barrel - pain in the jacksie
My wife has a more recent force lever where the barrel is different and the cable is captive and can't come out
anyone know if Rival shifters have been updated also?

I’ve a vague recollection that genuine Sram gear cables had a slightly larger barrel than others so they fitted the shifter? I may have got that wrong though as it’s been a while since I have had to think about such things!
Used to happen to me with rival 1x, I pinned it down to the b screw not being adjusted correctly. When I shifted there wasn't enough tension on the rear derailleur to have it moved, cable got slack and popped out.
