How long should it normally take to "warm up" and whats the best way to do it?

I would say for me it takes me about 30 mins to get into a long ride i.e. over 3 hours but yeh I guess about 15-20 mins easier riding as the start def helps - FWIW ๐Ÿ™‚

I tap out an easy gear for an hour. Hit training speed and gearing at about two hours, then the last two or three hours at pace.
I don't do any off bike stuff.

So after an hour pootling you feel you are warmed up?

I on Sunday I did about 50miles in total. The first 30 miles at a slowish pace (head wind) but with a couple of 25% climbs.

The last 20 miles home were when my legs felt warmed up

Usually about an hour. Tends to prevent the searing pain of ongoing hamstring/ITB/knee issues that come on when I welly it from the off.

When I was doing much more riding a year ago, it would be around an hour before the soreness would go from my legs allowing me to properly open up.

Tap out an easy gear? You forgot to say that you were then dancing on the pedals and finished on the rivet
Takes me about 40 minutes or so.

Headwinds knacker you out quicker and make me feel like I am unfit - you just have to go with it and put it down to the head wind ๐Ÿ™‚ I would say 30 mins for your body to wamr up and get into the swing of a long ride myself.

Yep. As an example, I did a 100k RT Sunday and as you know it was gale force and pouring, that got me round in 3.15 so about 19+ mph average.
I do about 40 minutes for a RR or about 5 for a CX as I can never find the field we're racing in.

10-15 minutes to my usual meet up place, by which time I'm pretty much ready to go, but ideally another 10-15 minutes to really get going properly.

For me it depends on conditions and how i feel. If i'm rested and not starting out into a headwind, i feel pretty good within maybe 10mins. If its day two of riding/i'm carrying a lot of fatigue, about 30mins maybe, or until after a good climb pushes my hr up. If its straight out the door and into a head wind all the way, most of the ride. ๐Ÿ‘ฟ
