New chain and cassette fitted, new cable too, yet stll crap indexing, jumping on the cassette underload. I've fettled for ages. The hanger appears to be straight, but in some gears the dérailleur arm seems to kick out slightly away from from the wheel rather than being vertical. Should it be vertical in all gears? New mech time?

Yup, should be vert all the time.
Try a new mech & have the hanger alignment checked.

Did you replace the gear outers?
Worn pivots on the rear mech.

Check that the last cable ferrule on the end of the cable outer is fully stuffed into the cable stop in the rear mech.
