My oldest and most loved (serious sentimental value!) frame has a rust pinhole on the seat tube (1mm diameter) and what I assume is tin worm rust (wiggly patterns) spreading under the paint around it.
It's not a bike I use for anything too testing (byways & bridleways) but I'd prefer it survived to ride, I'm not interested in wall frames.
Should I sand down, treat & fill like a car or something more specific? A full tube replacement would be pushing the economic / sentiment ratio to the extreme.

Maybe use it as a excuse for a frame refurb, get the frame stripped/blasted and if it is just a pinhole either a bit of braze or a blob of weld over it.
Smooth it down then get it powder coated and build up again.
