I get the same thing and my sports massage mate said it is a common problem with clip ins. But it may not always be down to the clips.

I have been told I have the tightest hamstrings he has ever had to deal with and this is causing no end of inner and outer knee pains. And with that it is starting to affect my back.

As you will know, if your cleats are not set up correctly for your feet you will get some discomfort. Sometimes on the outside of the foot but this can really play havoc with knees.

Do you have a good stretch before riding and are your cleats ofset correctly for each foot?

My knees are pretty sensitive to saddle height...too high or too low gives me grief...getting spd cleat angle can be important too (i.e pointing inwards/outwards) but I couldn't advise on this...

Forgot saddle. Been faffing with mine as I need a different height if I have pro pedal on. I find it causes me more hamstring pain if the saddle is too low and knee pain if too high.

How about seat forward and aft? Too far back and it cripples my knees.

Ride possition is a big faff isn't it?
They say to put you foot on the pedal in it's normal place, wind the crank round to 3 o'clock then check your knee is directly over the axel of the pedal.

If you don't get it from Flats then use flats.

I would get your cleat positioning checked-toe in/out/aft etc.

Also rest and see a GP or sports physio/advisor.

If nothing works get flats.

if you're at all worried i'd ignore all internet "advice" and go see a qualified specialist (one that understands cycling is good)
